Today’s class was nice and it helped me make my understanding even stronger but i feel we did not discuss much. We had to read about light last time and we did discuss nothing about that. Then we tend to forget by the next time we have class and then you say you did not read and stuff. I would have liked us to not discuss all the questions rather discuss about light. The speed of class was pretty much good.

Yes i gave my full attention to class and i feel i understood almost all when i left class.

Yes i used all my time productively. Just at the time you entered i did not do anything because i wanted to do BM and you did not allow me to do it. SO i got into an unwanted arguement or else i used my time nicely and did lots of maths and BM.

CLASS
I attended all classes unless I had a really important reason for not attending (serious illness etc.)
I came to classes on time (9:30 am or 11:15 am) without having to be "escorted" by Ma'am!
I had all my books, notebooks and supplies ready at the beginning of class so we could start class right
     away
I did not waste time by having to leave the classroom to retrieve my notebooks or other supplies
I paid attention and participated in class
I took really good, detailed notes in class
I kept my class notes safely after class and could find them at any time in the future to review, use for
     quizzes etc.
I did my best to focus and not have conversations with other students in the middle of class; Priya
Ma'am did not have to ask me to stop talking
I planned in advance so that I didn't need to use the washroom or get water during class
I make it a point to bring my class notebook to school everyday (even on non-class days) bc I know
that we may have extra classes during proxy periods etc

MISSED CLASSES
If I knew I'd miss class in advance, I communicated that to Priya Ma'am
If I missed class, I spoke with Priya Ma'am within a day or two about when to submit homework that
was due or take quizzes I missed
During my absence (due to illness, traveling etc.) I made my best attempt to work on maths
     homework
If I missed class, I sent Priya Ma'am an email letting her know when I thought I'd return to school and
how I'd like to make up the missed class material
If I missed class, I copied notes from my classmates as soon as I possibly could
If I missed class, I read my classmates notes from the missed classes and made my best attempt to
review missed material before attending the next class so I wouldn't fall behind
If I missed class, I planned ahead of time to see Priya Ma'am (during free periods, mornings, lunch
etc.) to review the missed material and ask for help
If I missed part of class for a school activity, I did my best to return to class as soon as I could

HOMEWORK COMPLETION
I made it a priority to invest the necessary study time and complete my homework
I did not "forget" to do my homework or wait until the last minute to start it
Since I know that homework is due every Monday, Wednesday and Friday (unless otherwise stated) I
checked the blog regularly for homework assignments
I managed my time well and planned my study hours so I could complete my homework on time
If was unable to access the internet, I called a friend to ask for the homework assignments
I referred to my class notes to refresh the topics in my mind and help me with homework assignments
I attempted every sum assigned even if I didn't know how to solve all of them
If I was unsure about how to solve a problem, I at least made an attempt by writing down any needed
     formulas etc.
I asked for help with sums I didn't understand (during free periods or over chat)
If I struggled with many sums on a homework assignment, I emailed Ma'am or told her over IM before
the day the assignment was due
I make it a regular habit to ask for help with sums during free periods, over IM or through email
I have file/folder to store my homework
I keep all homework assignments safely so that in the future, I could find any of my previous homework
     assignments to refer to

HOMEWORK SUBMISSION
I submitted all homework on time (even if it wasn't completely finished)
I submitted all homework in the physics/math envelope in the physics lab on days when Priya Ma'am
wasn't in school (without haven't to be asked or reminded)
All homework I submitted was on loose sheets stapled (there's a stapler in the library) with my name
and the date the assignment was due
I came to class with my homework already stapled and ready to submit at the very beginning
If I promised to submit something by lunch, free period etc. I made sure to hunt Priya Ma'am down and
     submit it J
If I did not even begin an assignment by the time it was due, I submitted the assignment the very next
day unless discussed otherwise with Priya Ma'am
If I completed only part of an assignment, I submitted what I had finished on the day it was due and
submitted the remainder of the assignment as soon as I finished it
If I was absent when an assignment was due, I submitted the hw on the first day I returned to school
(unless I discussed otherwise with Priya Ma'am)

FREE PERIODS
I came to see Priya Ma'am during all free periods that weren't spent on other school activities (CAS,
     band etc.)
I made an honest attempt to get help with Maths and Physics during free periods
If I needed to work on other subjects during free periods, I let Priya Ma'am know
I didn't waste time during free periods with talking or rough-housing too much
At the end of each free period, I accomplished a lot in Maths/Physics or whichever subjects I worked on

QUIZZES
Since I know I have a quiz each week (unless announced otherwise), I asked in advance about the quiz
     topics
I prepared well for my quizzes by studying and asking questions
I let Priya Ma'am know in advance when I wanted to take my quiz and informed her if any changes
     occurred
I came prepared for the quiz with my class notebook, laptop, loose papers and pens/pencils/GDC (if
     needed)
I brought all class notes, my own personal notes and old homework assignments to help me with my
     quizzes
If I was concerned about my understanding of any of the quiz material, I shared my concerns with Priya
     Ma'am
I was 100% honest during my quizzes and I did not ask for help or discuss any of the questions with my
     classmates
If I failed a previous quiz, I asked to see the quiz during free periods and asked questions so I could learn
     the concepts
MORNINGS, IN-BETWEEN CLASS TIME and GENERAL FREE MOMENTS DURING THE DAY
I used my morning times 9-9:30 efficiently and worked hard; if the DP class was too noisy or distracting, I
went outside or to the library to work
By 9:30 each day, I honestly felt that I accomplished a lot for 30 minutes of work (in any of my subjects
     or in school activities)
I used my "between class" times efficiently;  Even if I had only 5 minutes, I used it to plan the rest of my
day, work on a sum, ask for help with something etc.
I honestly feel that I used every spare minute efficiently and that by doing this I saved time I'd otherwise
have to spend working at home
I creatively used any additional time during the day to take care of classwork, study etc:
while waiting for a teacher to arrive for class, while sitting around for any school activity
I did my best overall each day to not be sucked into the many distractions around me: surfing the net,
facebook, music, talking (alll the time! Some talking is natural), rough-housing, pouring water on
     people, Iming
If my classmates were noisy in the DP room at any time, I went outside so I could focus on my work
I allowed myself no more than one hour total each day (mornings/lunch included) for talking/hanging
out during the school day and I spent the rest of my time productively

PROXY LECTURES
I honestly feel that I was respectful towards any proxy teachers I had and I followed their directions
    wholeheartedly
If the proxy teacher told us to work independently on assignments, I did so quietly for the most part and
didn't get sucked into playing around with my classmates
If the proxy teacher presented us with a new lesson, I gave it my best attention and participated (even if
     I didn't want to)
At the end of the period, the teacher left the classroom happy with my work and behavior; he/she
suffered no adverse health reactions due to my behavior: laryngitis from screaming too much,
skyrocketing blood pressure, a nervous breakdown etc

STUDY HABITS AT HOME
I review class notes at home and use them to help  me with homework sums
I review my class notes and write addional notes in the margins as I come to understand them better.  I
     study these notes before quizzes
If I have doubts about any sums, I seek help by asking Priya Ma'am over chat, asking my classmates or
someone else who can help me
I plan my study time in advance - if I know I will be busy with an activity on a particular night, I make
plans to finished needed work before that activity
I make it a habit to turn off distractions (T.V, IM, facebook etc.) while studying

ORGANIZATION SKILLS
I keep a file/folder with all my homework assignments; If asked, I could find any hw assignment I've been
given since Jan in this file
I keep an organized notebook for taking class notes.  The notes are in order with the date written on
them and I can look back on what we've done in any class
I keep a file wih loose sheets for doing my homework assignments and taking my quizzes                                                                                                                                                                                          I was able to produce any materials necessary during the week as needed: my notebook and loose
sheets for the quiz, notebook for taking notes in class

I bring my class notebook, file with homework and file with loose sheets on a daily basis to school (even on non-class days so I can work during free periods)
I pack my bags for school in advance and don't forget anything at home (class notebooks, homework,
     textbooks etc)

GOAL SETTING
I set high goals for myself at school and in life
My goals are realistic and achievable - I know that with the right amount of effort on my part they can be
     achieved
I break down large abstract goals (ex. Gain admission to Oxford) into tangible steps (ex. Perform well in
maths by investing 14 hours study time a week)
I think about my goals at least a few times a week
I think about the steps I need to take to achieve my goals at least a few times a week
I push myself to make improvements in my study habits, time management etc.
I take charge of my own education
I think for myself and am not overly influenced or distracted by my classmates
I take initiative on my own and do things without having to be prodded by my parents, teachers etc.
I know that I will be mentally and emotionally ready to move out on my own next year to attend a
     University
